Understanding Osteoarthritis

Osteoarthritis is a debilitating disease of the joints that affects a huge proportion of the population.  Up to 85% of people over the age of 70 and 20% of the general population are affected by this painful disease. Healthy joints are protected and cushioned by layers of connective tissue called cartilage that allows them to move freely without any grinding or contact between the bone surfaces themselves.  A special fluid in the joint capsule, called synovial fluid, is also extremely important for joint function. It acts as a natural lubricant and cushions joint movement. In patients with osteoarthritis there is a reduction in synovial fluid and a loss of cartilage that eventually leads to cartilage degeneration. This results in severe joint pain, inflammation, stiffness and an eventual loss of mobility.

The most common treatment for osteoarthritis involves anti-inflammatory drugs that treat the symptoms, but do not target the root cause of arthritis. Hyaluronic Acid

Hyaluronic acid is an essential component of the synovial fluid, which is required for proper joint lubrication and function. It also has anti-inflammatory effects.  Patients with osteoarthritis have been shown to have reduced hyaluronic acid content in their joints.  Normal synovial fluid contains 3-4 mg/ml of hyaluronic acid. Hyaluronic acid helps protect joints by augmenting the viscosity of the synovial fluid, by increasing the elasticity of the cartilage found in joints and because of its anti-inflammatory effect. Supplementation with hyaluronic acid reduces pain and improves function in patients with osteoarthritis of the knee. These effects are thought to be attributable to the return of a more normal synovial fluid, improved viscoelasticity, improved cartilage biosynthesis, anti-inflammatory and analgesic effects.

Boswellia serrata

Extracts of the plant Boswellia serrata have been noted for their anti-inflammatory, anti-arthritic and analgesic properties. Boswellia extracts have even been shown to be more effective than the anti-inflammatory drug valdecoxib for improving arthritis symptoms over a six month period. Furthermore, unlike anti-inflammatory drugs, the positive effects of Boswellia persist even after intake of the herb is stopped. This suggests that Boswellia may be targeting the source of osteoarthritis, not just controlling the symptoms.

Devil’s Claw

Extracts from the African plant Devil’s Claw contain harpogasides – active compounds with potent anti-inflammatory, analgesic and anti-arthritic actions. Clinical trials with devil’s claw have had very promising results.  Devil’s claw has been shown to significantly reduce the pain and stiffness associated with arthritis.  It reduces inflammation, helps promote cartilage regeneration and increases the production of hyaluronic acid. In one study, 171 out of 222 patients taking devil’s claw reported a beneficial effect, and showed significant improvements in pain, function and stiffness scores.

Mechanisms of Action

The ingredients contained in Advanced Joint Support work via a variety of mechanisms to improve the symptoms and debilitating effects associated with osteoarthritis.  Devil’s claw and Boswellia serrata act to decrease inflammation by blocking key enzymes, like cyclo-oxygenase-2 (COX-2) and lipoxygenase that are involved in stimulating the inflammatory process. They also act to suppress certain pro-inflammatory signaling and molecules like leukotrienes. Boswellia serrata and devil’s claw have an even broader mechanism of action, inhibiting signaling molecules and enzymes that break down collagen and proteins called glucosaminoglycans (GAGs). Collagen and GAGs are both critical components of cartilage. Devil’s claw has even been shown to increase GAG synthesis, promoting the formation of new cartilage.

In an open, randomized, controlled clinical trial of Boswellia serrata extract as compared to valdecoxib (a Cox-2 Inhibitor) in patients with osteoarthritis of the knee it was found that in those taking the boswellia, the pain, stiffness, difficulty in performing daily activities showed statistically significant improvement with two months of therapy which even lasted till one month after stopping the intervention. Those taking Boswellia showed a slower onset of action but the effect persisted even after stopping therapy while the action of valdecoxib became evident faster but waned rapidly after stopping the treatment.

In a study on the effectiveness and safety of Devil’s Claw taken by patients with general rheumatic disorders it was demonstrated that there were statistically significant improvements in patient assessment of global pain, stiffness and function. There were also statistically significant reductions in mean pain scores for hand, wrist, elbow, shoulder, hip, knee and back pain. Quality of life measurements were also significantly increased and 60% patients either reduced or stopped concomitant pain medication. Devil’s Claw is an effective and well-tolerated serious treatment option for mild to moderate degenerative rheumatic disorders providing improved quality of life measure.

Objective: To compare the efficacy, safety and tolerability of Boswellia serrata extract (BSE) in osteoarthritis (OA) knee with valdecoxib, a selective COX-2 inhibitor.

Materials and Methods: In a randomized, prospective, open-label, comparative study the efficacy, safety and tolerability of BSE was compared with valdecoxib in 66 patients of OA of knee for six months. The patients were assessed by WOMAC scale at baseline and thereafter at monthly interval till 1 month after drug discontinuation. Antero-posterior radiographs of affected knee joint were taken at baseline and after 6 months.

Results: In BSE group the pain, stiffness, difficulty in performing daily activities showed statistically significant improvement with two months of therapy which even lasted till one month after stopping the intervention. In valdecoxib group the statistically significant improvement in all parameters was reported after one month of therapy but the effect persisted only as long as drug therapy continued. Three patients from BSE group and two from valdecoxib group complained of acidity. One patient from BSE group complained of diarrhea and abdominal cramps.

Conclusion: BSE showed a slower onset of action but the effect persisted even after stopping therapy while the action of valdecoxib became evident faster but waned rapidly after stopping the treatment.

Arthritis and other rheumatic conditions (AORC) are the leading cause of disability, are associated with poor quality of life and incur considerable direct and indirect costs. It is considered that the instance of AORC will continue to increase. To assess the effectiveness, safety and tolerability of Harpagophytum (Bioforce) in the treatment of AORC, a single group open study of 8 weeks duration (259 patients) was performed in the United Kingdom. Effectiveness was assessed by numeric rating scales, the Western Ontario and McMasters Universities Osteoarthritis (WOMAC) Index and the Algofunctional Hand Osteoarthritis Index. Tolerance was measured by a numeric rating scale and safety by self-reporting, blood analysis and liver function tests. Quality of life was measured by SF-12 questionnaire. There were statistically significant (p < 0.0001) improvements in patient assessment of global pain, stiffness and function. There were also statistically significant reductions in mean pain scores for hand, wrist, elbow, shoulder, hip, knee and back pain. Quality of life measurements (SF-12) were significantly increased from baseline and 60% patients either reduced or stopped concomitant pain medication. Harpagophytum is an effective and well-tolerated serious treatment option for mild to moderate degenerative rheumatic disorders providing improved quality of life measure.
